Register as a New Patient
All newly registered patients will be asked to complete a simple questionnaire.
It is our practice policy to allow patients to choose whichever doctor they wish to attend although you will be allocated a named GP when you join the practice.
Continuity of care works best if you try to stay with the same doctor for a particular medical issue.
Whenever possible you will be seen on the day you request providing the doctor is available.

Our boundary includes the new estates of Kingshill Grange and Hughenden Quarter. Patients living in the Kingshills and Holmer Green are currently being registered at our Prestwood site.

If you move house or change your name or telephone number please let us know by telling Reception or writing to us. It is very important that we are able to contact you.
